Terms and Conditions

Dr. Rachell Richman’s Dental Savings Plan for a period of 12 month. Activation requires full payment of annual cost. I understand there is no early cancellation or termination option. Fee for services are due at the time of service. Fees charged by labs require a 50% down payment. Failure to provide payment at time of service will result in being charged usual and customary fee. By enrolling I agree with the terms and conditions of the plan. 

Limitations and Exclusions 

This plan only includes services at Dr. Rachell Richman’s office. It cannot be used at any other dental office or specialist office.  No other discounts will be given in conjunction with the plan, e.g. Senior Discount, Cash Discount. If payment is made with Care Credit, plan service discounts will be reduced by 7%.
